The ruler of the crown-lands and the iron thrones is both king and emperor of Westeros. Lord Paramount is the warden of such region. Lord paramount is the person in control of a region (The North, Reach etc) under the authority of the Iron throne. The Highlordship is the title given to a duke who controls counties. The lordship is the title given to a lord who controls the land in question.

The county is called The Dreadfort and so is its county capital, however other fiefs are in the county that Roose does not control, yet through laws he controls the tax's the lord of the lands pay and the levy they must supply in time of war.

This means he has direct control of The Dreadfort and indirect control of the surrounding lands and titles. So for example lets take Roose Bolton at the beginning of a Song of Fire and Ice, he is Highlord of The Dreadfort, his main title is Highlordship of The Dreadfort, his secondary title is The Dreadfort. In terms of mod this follows suit, a lord is anyone who has a landed title in a county or even multiple titles however does not have highlord ship of the region.
